1. How do actual duels within the story work? Are they always moderated or do some people decide on predetermined outcomes before RPing?
2.What are the rules regarding time? Obviously a character can't be two places at once. How many threads with the same character are normally going at one time? Just one?
Also, it would be good if I could get a bit of feedback on my app. I'm not here asking for full reviews, but it would be nice if I could find out if my stats need adjusting or what areas of my bio need more detail (there are probably several)

1. Depends. If both players involved can come to an agreement on the outcome, then its fine. (this is the road I preferred.) However, if you both can't agree on who damaged who, then yes moderator comes in.
2. Liquid time all around. You can have a character in two threads I think, but you just decide which came first and your good.
Bear in mind.... I'm not 100% on all of this, but this is how it was explained to me.

1. Only the duels in the Practice Duels board are moderated. Fights within the actual story would be determined by the participants. Unless it's a huge, site-wide battle (i.e. Yaga Minor) where the staff have a predetermined outcome already in mind. But if you mean just a regular one-on-one duel, then it's up to the two members playing those two characters to determine what happens. Moderators generally aren't needed in this case.
2. One character is allowed to be in as many threads as you want, as long as you can keep track of the time line/order of occurrence and of the threads themselves. I've seen a few members on the site who have one character in half a dozen or more threads.
As for feedback, you just have to be patient. Generally if you keep mentioning it, the mods tend to wait longer. But if you ask an NGT or another member who likes doing reviews politely, they might help you out.
I hope this helps?

To confirm the first point as I believe the others have been covered.
We have pre-determined outcomes for major battles. Individual fights are left down to the users. However, if both participants wish to have a judged duel then they may do so, thought generally we like to avoid this as coming up with the story between your fellow members more often than not leads to greater story-telling results.

A note on the 2nd: Bewaaaare of continuuuuity!
Especially if you're establishing new history for a character that wasn't covered or conflicts with the bio.
